Nurse/Specialist Nurse for Gränna

In this role, you will have the opportunity to follow patients over time and actively contribute to safe and person-centered care.

Your new job

As a nurse, you will be responsible for both planned and acute health and medical care interventions in individuals' homes, primarily within special housing but also within regular housing. You will collaborate with primary and specialist care to tailor interventions according to individual needs. The work is based on person-centered care, where the patient's influence, participation, and safety at home are the focus. Here, you will have the opportunity to follow patients for an extended period.

Your role includes independently making medical assessments, performing treatments and interventions. Documentation, medication management, and educational efforts are other central parts of your work. You will collaborate both internally and externally to ensure continuity, efficiency, quality, and patient safety.

Your qualifications

  • We are looking for a licensed nurse
  • It is an advantage if you have specialist training in elderly care, palliative care, psychiatry, or as a district nurse
  • We view positively if you have experience working in care and nursing
  • A valid driver's license is required as a car is used in the job
  • You have good knowledge of Swedish, both spoken and written, as well as good computer skills

Since the work takes place in home environments, good physical mobility and the ability to adapt your working methods to existing conditions are also required.

You are an empathetic and attentive person who meets patients and relatives with care. As days can vary, you need to be flexible while also having the ability to create structure. You communicate in a considerate and respectful manner that contributes to safety and quality. With good judgment and accuracy, you apply your knowledge in practice to provide the best possible care.

Welcome to us

The Health and Medical Care function (HS) is part of the social administration in Jönköping Municipality and consists of 13 HS teams and one Home team. We have specialist orientations in psychiatry, palliative care, home rehabilitation, dementia, and functional support. Together, we are about 460 dedicated employees.

The Health and Medical Care Team 1 has a catchment area of Gränna, including rural areas, Visingsö, and Ölmstad. In the catchment area, there are two nursing homes; Annero nursing home and Gränna nursing home. The workgroup is based in an office in central Gränna and consists of nurses, specialist nurses, occupational therapists, physiotherapists, and nursing assistants.

Good to know

The recruitment refers to a permanent position. Jönköping Municipality works with full-time as the norm and always offers 100% employment.

The working hours are primarily during the day, with some evening and weekend work included (occurring approximately every fifth week).

Selection takes place continuously during the application period, and interviews are planned to be held during weeks 1 and 2.
